We have been very impressed with Stonemere Rehabilitation Center on Lebanon Rd in Frisco, TX, working closely with the professional staff and specifically, Kim Ramirez, the Marketing Director for Admissions and Referrals. As in home care specialists, our joint goal is to work closely to reduce hospital readmission and increase wellness and safety for residents and home bound clients. We coordinate care together to connect senior patients with in home care services prior to discharge, so stressed is reduced and the family has an experienced caregiver ready to pack up the room, drive them home, unpack, make meals, pick up medications at the pharmacy, to continue the optimal recovery process at home.
